CLINIC FORM

One form per horse/rider combo. Preferred method of payment is check mailed with clinic form and BSHP release. PayPal is acceptable, but there is a 3% fee.

Payment for your clinic space can be made in two ways; 1) check made out to OCEN, LLC/Natalie Sullivan and mailed with clinic form or 2) paid via PayPal below (includes processing fee).
